Travel Services

Important –  You need to submit a travel risk assessment form to the surgery a minimum of 6 weeks prior to travel,  allowing the nurses time to assess what you need and arrange the vaccinations.  If you are travelling in less than 6 weeks you will be advised to speak to a private travel clinic.

If you are travelling abroad and would like to discuss your travel vaccination requirements please click travel risk assessment form Swavesey Surgery to download the travel questionnaire.

The top part of questionnaire needs to be completed and returned to the surgery you can email this to us admin.swaveseysurgery@nhs.net

You will be contacted by the surgery once your travel risk assessment form has been reviewed and advised on what appointments are required.

It would be useful if you could take a look at the Travel Health website http://www.travelhealthpro.org.uk before your appointment.  This website gives information about the latest travel alerts and you can also access the vaccination advice for the country you are travelling to.  This will enable you to make informed decisions during the consultation.  Please be aware that some vaccinations require a series of injections at intervals prior to travel.

If you require urgent advice/vaccination, we will do our best to offer you an appointment, but if this is not possible please see the Masta website https://www.masta-travel-health.com to find a local clinic.
