Veterans Accreditation

Swavesey Surgery is an accredited Veteran Friendly GP Practice. We are proud to support our Armed Forces community. As a Veteran Friendly GP Practice we:

  • Have a clinical lead for veteran health Dr Anil Jain
  • Ask patients ‘Have you served?’ to identify veteran patients
  • Support veteran patients to access dedicated health services
  • Undertake specialist training to meet the health commitments of the Armed Forces Covenant

If you are a veteran patient, please let us know that you have served so that we can make sure that we understand your health needs. Nearly 1,100 practices are now Veteran Friendly accredited. To find out more about the Veteran Friendly Practices accreditation scheme, visit rcgp.org.uk/veterans .  The Veteran Friendly Practices accreditation scheme is run by the Royal College of General Practitioners in partnership with NHS England and NHS Improvement.

Our expectations of you

We aim to treat our patients courteously at all times and expect our patients to treat our staff in a similarly respectful way. We take seriously any threatening, abusive or violent behaviour against any of our staff or patients. If a patient is violent or abusive, they will be warned to stop their behaviour. If they persist, we may exercise our right to take action to have them removed, immediately if necessary, from our list of patients.

You can search for your postcode by clicking the magnifier in the top right corner of the map, this will indicate whether you live within the boundary or not.

Named GP

In line with the current government initiative and to comply with our NHS contract all patients are now given a named accountable GP.

Newly registered patients will be allocated a named GP on registration.

Your named GP will be Dr Anil Jain, but please note that this does not mean that you cannot see the GP of your choice and can book appointments with all GP’s.

Accessing someone else's information

A competent patient can choose and consent to allow access to relatives and/or carers. The Consent to proxy access to GP online services form must be completed. The patient will authorise a Proxy Application in the following circumstances:

A patient who has been deemed as competent has authorised and consented to online access.

Circumstances when the practice will consider authorising proxy access when the patient does NOT have capacity to consent. This will need to be discussed with the practice on an individual basis and maybe granted on a case by case basis. Ensure the level of access granted to the Proxy is appropriate
