Online Services

You can contact us about a medical, administrative or prescription issue by clicking HERE.

This short online form is the quickest and simplest way for the majority of our patients to access or clinical team for assistance. Our practice phone lines remain open for anyone with accessibility needs who might find difficulty completing this form.

Please do not use this online form for urgent or emergency requests. The submitted forms will only be read during office hours. The forms will not be read on the weekends (Saturdays and Sundays) or on bank holidays. They will not be read Monday-Friday between the hours of 1800 – 0900.

Please contact 111 during out of hours or 999 if this is an emergency.

ONLINE ACCESS

We would encourage you to use online facilities to order repeat prescriptions and manage appointments where you can.

We have various options for patients to access online information:

The NHS APP – This is used with a smartphone or tablet – No need to contact the surgery, you can set this up by downloading the NHS App, or open the NHS website in a web browser. Owned and run by the NHS, your NHS account is a simple and secure way to access a range of NHS services online.

SystmOnline –  This is set up using your email address and a generated password –
You will need to come into the surgery to complete a registration form, or you can download it here to complete and then bring it to the surgery; you will be asked to provide two forms of identification with photo ID if you are not well known to the member of staff.

The NHS App

Owned and run by the NHS, the NHS App is the simple and secure way to access a range of NHS services on your smartphone or tablet.

Use the NHS App to:

  • book appointments – search for, book and cancel appointments at your GP surgery, and see details of your upcoming and past appointments
  • order repeat prescriptions – see your available medicines and request a new repeat prescription
  • check your symptoms – search trusted NHS information and advice on hundreds of conditions and treatments, and get instant advice or medical help near you
  • view your medical record – securely access your GP medical record
  • register your organ donation decision – choose to donate some or all of your organs and check your registered decision
  • find out how the NHS uses your data – choose if data from your health records is shared for research and planning

SystmOnline

Swavesey Surgery fully supports Patient Online, an NHS initiative to offer online services such as booking appointments, ordering repeat medications and access to your health record. We hope this makes managing your own health needs as easy and convenient as possible.

Booking Appointments:

You can book the following types of appointments online:

  • Routine GP appointments
  • Telephone GP appointments
  • Phlebotomy (blood test) appointments

Appointments for our nurses are not currently available online because of the significant variation in length of time for each appointment dependent on the clinical need. These are best booked by contacting our reception team.

Cancelling Appointments:

Appointments can only be cancelled if they are more than one hour ahead. If you need to cancel an appointment with less than an hour’s notice, please contact the reception team.

Proxy Access

A competent patient can choose and consent to allow access to relatives and/or carers. The Consent to proxy access to GP online services form must be completed. The patient will authorise a Proxy Application in the following circumstances:

A patient who has been deemed as competent has authorised and consented to online access.

Circumstances when the practice will consider authorising proxy access when the patient does NOT have capacity to consent. This will need to be discussed with the practice on an individual basis and maybe granted on a case by case basis. Ensure the level of access granted to the Proxy is appropriate
