In order to register with us you can attend the surgery for a new patient pack, or you can download the following forms to complete and bring with you to save time.

Named GP
In line with the current government initiative and to comply with our NHS contract all patients are now given a named accountable GP.
Newly registered patients will be allocated a named GP on registration.
You will be registered with Dr Anil Jain, but please note that this does not mean that you cannot see the GP of your choice and can book appointments with all GP’s.
On-line access
To register for on-line access to book appointments and order repeat medication and access to your health record you will need to bring tow forms of ID to the surgery.
1. Photo ID (passport or driving licence)
2. Proof of address (recent bill or bank statement)

Patients with particular needs
The surgery is accessible to patients using a wheelchair although it is awkward. Please telephone ahead of your appointment for any help you might need.
Patient confidentiality
We respect your right to privacy and keep all your health information confidential and secure. It is important that the NHS keeps accurate and up-to-date records about your health and treatment so that those treating you can give you the best possible advice and care.
This information is only available to those involved in your care and you should never be asked for personal medical information by anyone not involved in your care. You have a right to know what information we hold about you. If you would like to see your records, please call our practice manager or speak to your doctor at your appointment.
Our expectations of you
We aim to treat our patients courteously at all times and expect our patients to treat our staff in a similarly respectful way. We take seriously any threatening, abusive or violent behaviour against any of our staff or patients. If a patient is violent or abusive, they will be warned to stop their behaviour. If they persist, we may exercise our right to take action to have them removed, immediately if necessary, from our list of patients.
